About Us

Unite South Dakota is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite South Dakota is sponsored by CyncHealth. The network is supported by a South Dakota-based Unite Us team focused on community engagement, network health and optimization, and customer success. 

There may be a cost associated with data-sharing, but no additional charges to join Unite South Dakota.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Helping people get well from addiction is the most important work we do. We were eager to join the Unite Us network to extend our services across the region. We often make referrals to outside entities – this is an important step to ensuring we’re meeting the needs of our members

Pathways joined Unite South Dakota in order to be more efficient in connecting our clients with the many resources available in our community. Our ultimate goal is to break the cycle of homelessness in those we serve. An individual has a greater chance at success when they receive a seamless transition to the services they need, which Unite Us provides an opportunity for.

Network partners are now able to send and receive secure, electronic referrals in Arizona, Arkansas, Colorado, Illinois, Iowa, Kansas, Louisiana, Minnesota, Mississippi, Missouri, Montana, Nebraska, Nevada, North Dakota, Oklahoma, South Dakota, Texas, Wisconsin, and Wyoming through the Unite Us Platform, which ensures communities and individuals can seamlessly access much-needed resources and services across state lines.
